Ozempic Face After Weight Loss: Is Lifting Alone Enough?

Facial hollowing, prominent cheekbones and sagging can appear together after significant weight loss. Natural correction often requires more than lifting alone.

After significant weight loss with Ozempic, Wegovy, Mounjaro or other GLP-1-based medications, some people notice that their face looks very different from before.

The cheeks may look flatter, the area below the cheekbones may become hollow, and the cheekbones themselves can suddenly appear more prominent. At the same time, the jawline or lower face may begin to look softer or more saggy.

These changes are commonly described as “Ozempic face.”

Ozempic face is not one specific facial shape caused directly by a medication. It is better understood as a change in facial proportions that can become noticeable after substantial weight loss, particularly when facial volume decreases and skin laxity becomes more visible.

Ozempic face is not only about sagging

When the jawline becomes softer after weight loss, it is easy to assume that a stronger lifting treatment is all that is needed.

In many cases, however, more than one change has occurred. The lower face may show laxity while the midface and lateral cheeks have lost volume.

Hollowing below the cheekbones can make the cheekbones look sharper even though the bone itself has not changed. Loss of volume around the anterior cheek or lateral cheek can also make the nasolabial area and lower face appear more pronounced.

This is why lifting alone may not always be enough. Some areas need lifting, while other areas may need softer volume or collagen support to restore a natural facial contour.

The key question is not simply “Which lifting device is strongest?” It is “Where does the face need lifting, and where does it need volume support?”

Lifting still matters when the lower face begins to sag

When weight loss reveals laxity around the jawline, mouth corners or lower face, lifting can be an important part of treatment.

When the cheeks are hollow, Gouri can complement lifting

In these cases, Gouri can be used together with lifting.

Many people are already familiar with PCL, or polycaprolactone, through collagen-stimulating injectables such as Ellansé.

Gouri has an important formulation difference. It is a fully liquid, microparticle-free PCL injectable, rather than a conventional PCL filler containing suspended microspheres.

Because the PCL is delivered in a liquid form without solid microparticles, it is designed to distribute more diffusely through the treated tissue. This makes it suitable when the aim is gradual collagen stimulation and a softer, more natural contour rather than creating a sharply filled or projected area.

The absence of PCL microparticles also reduces the concern related specifically to particle accumulation that can contribute to palpable or visible nodules with particulate biostimulatory fillers. This does not mean that any injectable is completely free of complications, but the liquid formulation is an important difference when planning treatment for broad facial hollowing.

HILO WAVE uses high- and low-molecular-weight hyaluronic acid

Another option for facial hollowing and skin quality is HILO WAVE.

For international residents who are familiar with Profhilo, the concept may sound familiar. HILO WAVE is a different product, but it also uses a combination of high- and low-molecular-weight hyaluronic acid.

HILO WAVE is useful when the goal is not to create obvious filler-like projection, but to improve hydration, skin quality, elasticity and soft-tissue contour.

After weight loss, the facial curve between the cheekbone, anterior cheek, lateral cheek, nasolabial area and lower face can become less smooth. HILO WAVE can be incorporated into treatment when softer support is needed in these areas.

Gouri and HILO WAVE are used for different reasons

Gouri and HILO WAVE are not interchangeable treatments.

HILO WAVE combines high- and low-molecular-weight HA. It is useful when hydration, skin quality, elasticity and softer contour support are priorities.

The choice depends on where volume has been lost, how much hollowing has developed and how the surrounding facial contour has changed. In some faces one injectable may be sufficient, while in others lifting and injectable treatment work better together.

The most important part is reading the face correctly

Two people can lose a similar amount of weight and develop very different facial changes.

One person may lose most of the visible volume from the lateral cheeks. Another may develop deeper nasolabial folds and lower-face laxity. Someone else may notice prominent cheekbones, hollow cheeks and a sharper transition between the midface and jawline.

Can I have lifting treatment while I am still losing weight?
